What is Mini Gastric Bypass

There is nothing “mini” about this gastric bypass. The stomach is first divided to create a small and long “sleeve” pouch to reduce stomach capacity to store food. Then we bypass a significant length of small bowel (150 to 200cm) and connect them back to the divided stomach to create malabsorption (see picture below). The whole procedure is done through laparoscopic (key-hole) technique.

Why chose “mini” gastric bypass over the regular Roux-en- Y bypass?
  1. Multiple studies have showed similar weight reduction compared to regular bypass, may be better.
  2. It provides similar reduction or improvement of diabetes mellitus conditions.
  3. It is proven to have less surgical complications.
  4. It is easier to reverse the bypass should the needs arise.
Why is it not popular in United States of America?

Most bariatric surgeries in USA are covered by medical insurance. This is relatively new procedure compared to the regular bypass and it is not covered by most insurance companies. Therefore, there is less demand from patients. We predict mini gastric bypass will gain significant amount of attentions in years to come because of its simplicity, efficacy and safety.

Why should patients seriously consider mini gastric bypass?

Gastric bypass is proven a better long term solution for diabetes mellitus control compared to medical therapy. Patients who are obese and diabetic should at least discuss with their doctors about surgery for diabesity (diabetes + obesity). If mini gastric bypass is proven to be simpler, safer, and at the same time providing similar results compared to regular Roux-en- Y gastric bypass, there is no good reason for patients to consider something more dangerous and riskier!

What are the long term risks involved?

Patients may develop nutrients deficiency, such as, protein, iron, and calcium deficiencies. It is paramount for the patients to be follow-up by their doctors to monitor progress. Daily vitamins and mineral, dietary supplements are important.

Excessive weight loss or insufficient weight loss may occur. These are problem that can be corrected through lifestyle intervention or surgical changes especially in mini gastric bypass.

Ulcers can happen at the connection of stomach to small bowel. This happens mainly in smokers and those who regularly consume NSAIDs medication.

What is the best operation for me?

Each patient is different. You shall discuss all options with your surgeon, whether it is gastric banding, sleeve gastrectomy, mini gastric bypass, or intra-gastric balloon. Each of us accept different levels of risk, therefore, it is paramount to discuss what are available before making your decision.
